Accreditation audits can be high stakes, especially for Community Managed Organisations (CMOs) navigating the NSQMHCMO standards for the first time.
Knowing what not to do can be just as important as knowing what to do. Here are the top five mistakes organisations make and the strategies to avoid them.
- Treating Accreditation Like a One Time Event
One of the most common mistakes is focusing solely on preparing for the audit, rather than embedding the standards into daily practice.
Solution: Implement a year round quality and safety calendar.
Schedule internal audits, policy reviews, and staff training regularly, not just ahead of an external assessment.
- Inadequate Consumer Participation
Does your governing body know how your service users have been involved in service design?
NSQMHCMO places a strong emphasis on consumer voice. Services often fall short by not involving people with lived experience in service design, feedback, or quality improvement.
Solution: Form a lived experience advisory group. Invite feedback on your policies and procedures. Involve consumers in staff recruitment and training.
- Poor Documentation and Version Control
Auditors often find disorganised documentation, outdated policies, and inconsistent records. If it’s not documented, it didn’t happen.
Solution:
- Create a centralised document management system
- Keep all documents version controlled and date stamped
- Train staff on correct record keeping
- Not Engaging Staff Early
If staff are unaware of the standards or how they apply, your service will struggle in an audit.
Solution:
- Involve staff early in the accreditation journey
- Use practical examples and scenarios in training
- Encourage staff to ask questions and raise concerns
- Ignoring Governance and Risk Systems
Many organisations do not have clear governance frameworks, risk registers, or incident review systems in place.
Solution:
- Develop and maintain a risk register
- Ensure your Board receives regular updates on clinical and organisational risks. Your Board should also provide acknowledgement and direction.
- Review incidents and near misses quarterly. What insights and improvements can be drawn from these? Report these back to your governing body.
Bonus Tip: Failing to Act on Past Recommendations
If your organisation has been audited before, under NSQHS, HSQF, or another framework, and hasn’t implemented the recommendations, it will count against you.
Solution: Keep an action register and document follow through.
NSQMHCMO compliance requires more than good intentions. Avoiding these five audit mistakes will help your organisation build a strong foundation for safe, quality mental health care and make your next audit a success.
